Welcome to Spurs Lane Medical Associates (SLMA), an award-winning Adult Primary Care Practice. At SLMA, we understand patients have different needs and expectations when it comes to their health. We provide two ways you can establish care with our practice, either with Concierge (Annual Fee) or Traditional Medical Care (no Annual Fee).
Regardless of which option you select, rest assured you will be treated with respect and provided the most up-to-date, compassionate healthcare from providers you can trust.
Spurs Lane Medical Associates is now accepting new patients and features the following models of care for your convenience.
Concierge Medicine
For an annual fee, you can become one of Dr James Vosberg’s concierge patients. This care model allows patients to have priority access, expedited scheduling, and extended appointment times with Dr. Vosberg and his staff. There is a dedicated phone line with texting capabilities exclusively for concierge patients. This phone access is available 24 hours a day. Dr. Vosberg and his staff can assist with arranging specialist and radiology appointments. In office and telemedicine visits are available, as well as calls returned personally by Dr. Vosberg if needed. Due to the highly personalized care, there is a limited number of patients that can be accepted under this concierge model.
SLMA offers a variety of payment plan options for the annual concierge fee. These options include:
- Pay the full amount on an annual basis
- Pay the full amount on a quarterly basis

Traditional Medicine
There is no annual fee required to access these providers. Dr. Michael T.Almquist and Christina Rodriguez, FNP are available to see patients on an as needed and appointment availability basis. These providers are available to see patients with no insurance on a fee for service basis, those with Employer or Group Based Health Insurance, Original Medicare, and select Medicare Advantage Plans (Part C).
Original Medicare and
Medicare Advantage Plans (Part C)
Our Clinic providers are in the network with Original Medicare and/or Medicare Advantage Plans (Part C). Dr. Michael T. Almquist and Christina Rodriguez, FNP are immediately accepting New Patients. We are in-network with Amerigroup (Wellpoint), Devoted, Cigna, Humana and UnitedHealthcare.
Employer and Group Based Health Insurance
Spurs Lane Medical Associates (SLMA) accepts most Commercial PPO and POS health insurance plans. Refer to the list below of accepted insurance.
All applicable fees, deductibles, co-insurance, or co-pays must be paid at the time of your service. Outstanding balances will be billed to you or can be handled via our finance office.
